Adult Partial
Hospitalization (REACH Program)
REACH stands for Relationships,
Encouragement, Accountability, Change and Hope.
REACH is a voluntary adult mental health program
that offers participants a structured environment in
a less restrictive and more flexible outpatient
setting. Some participants enter the REACH
Program directly from the community, while others
transition from the hospital. Individuals
participate five days a week, Monday through Friday
from 9 am to 3 pm, and return home at night and on
weekends. The REACH Program provides
individuals with a safe and interactive environment
to share and process feelings. Through education and
support, our team encourages participants to
reestablish and maintain relationships and
connections in their communities. Because
participants go home each afternoon, opportunities
to practice skills and process real life experiences
in groups the next day allow for consistent
reinforcement and skill development. Participants
are assessed daily during a group check-in.
Based on feedback from this morning session,
therapists customize groups to address individual’s
needs. Group therapy sessions frequently focus on:
-
Depression
-
Anger
-
Anxiety/Panic
-
Greif/Loss
-
Post Traumatic Stress
-
Boundaries
-
